Rename chrome_elf.dll to chrome_elf_required.dll.
Download chrome_elf.dll, blockthespot.dll, and config.ini from the release, and drop them in that same folder.
Reopen the app — ads should be gone.

BlockTheSpot is maintained by its own developer on GitHub, and that's the only place the actual patch files (chrome_elf.dll, blockthespot.dll, config.ini) should come from. Mirroring binaries on a third-party site — even a well-intentioned one — makes it harder for you to verify you're getting the real, unmodified files, so we simply don't.
The GitHub releases page is maintained by the actual developer — no re-uploads, no middlemen.
